Definition

A self reflection survey completed by a practitioner prior to a mentoring session to focus their attention on actively participating in the value exchange and enable the mentor to prepare with valuable contributions.

How to do a focus sheet

  1. Use a free survey form eg Google Forms

  2. Practitioner to complete >1 Business Day prior to the mentoring session

  3. All responses go to the mentor and practitioner eg Shared Google Sheet

Mentoring Focus Sheet Questions

You don't need to ask all of these questions, these are just some of the questions you could ask.

  • Date

  • Bring the Energy: What has been your BIGGEST win over the past month?

  • Be Outcomes Focused: What is the most important outcome for today's session for you?

  • Keep it Real: What's happening in your world?

  • Keep it Real: To what degree are you feeling a workflow vs feeling a workload in your role?

  • Keep it Real: To what degree are you enjoying coming to work?

  • Be Outcomes Focused: How can we better support you in creating more flow and enjoyment the coming month?

  • Be Outcomes Focused: To what degree are you clear about your role in the strategic direction of Clinic over the next 1 Year?

  • Be Outcomes Focused: What are your strategic / High impact priorities over the next 1 Month?

  • Keep it Real: If I (Name) had my magic wand, what would I change about my role or insert clinic name?

  • What is clinic's Core Purpose

  • What is central to your personal purpose / passion?

  • Be Outcomes Focused: How can we better align and infuse your passions / personal purpose with Clinic name core purpose?

  • Keep it Real: What parts of your role are you really enjoying and thriving with?

  • Keep it Real: What are the parts of your role that you loathe / they drain you?

  • Make it Happen: How are you going to simplify your role in the next month?

  • Keep it Real: To what degree do you feel your ideas are welcomed?

  • Keep it Real: To what degree do you feel like your feedback is taken onboard?

  • Keep it Real: What behaviours do you need to change so that we can achieve our mission over the next year?

  • Keep it Real: What behaviours need to change in our team so that we can achieve our mission over the next year?

  • Keep it Real: To what degree have you contributed your candid feedback and voiced your opinion to the people who need to hear it?

  • Bring the Energy: To what degree have you contributed your appreciation, gratitude and praise for the team members who need to hear it?

  • Keep it Real: To what degree have you played above the line in the last month?

  • Keep it Real: Tell me (the mentor) what I don’t want to hear but need to hear

  • Make it Happen: What skill or concept have you learnt recently?

  • Be Outcomes Focused: How have you / can you apply this in your role?

  • What was your #1 personal focus for the past month? (what area on your wheel of life/desire statement)

  • What did you achieve in this area of your life? What is your #1 personal focus for the NEXT month? (what area on your wheel of life/desire statement)

  • What will you achieve in this area of your life? How could we support you to achieve this?

  • What was your "one thing" action point from the previous session, and how is your progress with it?

  • What is one thing you will achieve to make this next month AWESOME?
